2023-2024 Lake Travis High School Student Registration Form

PLEASE COMPLETE ONE PER STUDENT

All parents must complete this registration form. 
Payment can be done online (preferred) or by cash/check at the end of this form. Please complete even if requesting a scholarship for choir fees.

Special Dietary Needs for Choir Provided Meals:
Allergies for Choir Provided Meals:

LTCP is not able to guarantee the safety of meals for food allergies or sensitivities. Parents may opt-out of meal plan for physician-recognized food allergies and MFS fees will be reduced. Please email us at ltshchoirtx@gmail.com with dietary concerns.

Photo Release Opt-Out:

All LTHS Choir students maybe included in photos on the website or in other social media or promotional material. If you do not want your student included in photos, please check the box above.

Choir Fee Payment:

While we prefer online payments, we recognize that some families prefer to pay with a physical form of payment. Please indicate your preference below.
