Choir Handbook Acknowledgement
Students and their families should review the current handbook. After reviewing it, a parent or guardian should submit the acknowledgement form for their student. This response is recorded separately from student registration and payment.

Open the six-page 2026–2027 LTHS Choir Handbook and review the program expectations, attendance policies, uniforms, grading, and student responsibilities.

Once you have read the handbook, please open this form to acknowledge that you have read and understand the contents of the handbook.
What the form records
The separate response log gives directors the information needed to identify each student's completed acknowledgement without changing registration records.
- Confirmation that the handbook was reviewed and understood
- Parent or guardian name and email address
- Student name and LTISD student ID number
- Automatic submission timestamp
